Dear Prospective Parent,

ongratulations... you're immediately so close to having your personal 'junior' come along and join the family. You have no doubt planned where 'junior' will definitely sleep in the house, re-decorated the room and are starting to plan your futures together!


However Wait....
As you still should get through your labor and childbirth event, do you often locate yourself asking one or more of the following questions regards these...

Hello there, After having qualified as a Registered Nurse in 1990, and practicing my skills for a number of 2 1/2 years, I made a decision to take on a new challenge and train as a Midwife/ Obstetric Nurse.

I chose Midwifery as opposed to other disciplines which I could have chosen, as I did not particularly desire to work with poorly children; I did not desire to study psychiatry (what is 'normal' after all?); and I did not desire to specialise in high dependency nursing... which to me, appeared to be taking greater responsibility for high-tech machinery and losing out on the 'communication' side of things using your patients.

Immediately I realise that you do not need to open your mouth to communicate using your fellow man (or woman), however this field of nursing did not appeal to me anyway.


I successfully qualified as a Midwife in 1994, and practiced my skills in under a busy obstetric/maternity unit, being responsible for the care of females and their partners throughout their pregnancies, labors and early days following the birth of their baby's, providing support and education where necessary. I was also rather priveledged to have a great deal of input with girls's partners... partners who could often be left out of the 'mix of things' as the majority of attention focuses on the female part of the duo, or the unborn fetus (the fetus becomes the 'baby' once born). Partners were unfortunately often left to fend for themselves during busy or stressful periods.


I was fortunate enough to meet a variety of moms and dads in all areas of maternity care... in clinics, antenatal wards, labor suites, post-natal wards and in under the community. I was therefore able to increase much in the way of professional information of the aspects of maternity care, however also and perhaps more essential, It allowed me to empathise with these individuals. Empathy is 1 of the most significant aspects of an expert healthcare member I believe. To actually 'feel' for the individual(s) who're suffering to a greater or lesser degree, allows you to better realise what they're going through at a given time.

Why the stress?

Well, it can be a stressful event for several reasons, not least for a lot females and their partners, they only have not been through the experience prior to... it's therefore stressful for the reason that they have not any prior experience of labor, and they therefore do not know what to expect or the way to cope with it!

They can locate things stressful for the reason that they do not have all the relevent info prior to they need it... they have a number of idea of what it's like, however not the 'whole' idea, there's a difference!

Experiencing labor is stressful not just for the female experiencing it, however also for her partner (whether male or female) who is the majority of often in attendance during this time. These individuals are often 'suffering in silence', besides their beloved who is enduring the labor. They can feel they have little understanding of events. Labor is especially stressful if it's a first labor... though having said that, subsequent labors can, and often are, dissimilar from those having gone prior to. They often do not turn out in ways which subsequent ones did... and this poses an interesting point.


The Importance knowledge And Practise
'Experience' is an important factor in any human event. When we have not experienced something prior to, we've no previous understanding of that event... it is a 'new' experience for us.

We cannot therefore adapt our previously learnt 'coping tactics' with the aim to get us through the event, and consequently we feel anxiety, nervousness and sometimes, downright terror!

However, when we've experienced a situation or event previously, we can call upon our past 'coping mechanisms' to gain ourselves through the present situation. We cope with situations which make us nervous by acting in certain ways... we adopt a certain behavior, and certain thought patterns.

These are subconsciously designed to protect us from harm, and to maintain us in under the 'comfort zone' of past (and familiar) experience. We hopefully come out the other side unharmed and all that more better for the experience. Much in the way of us being able to effectively cope with a given experience is dependent upon our being able to call upon our previous 'knowledge' of the experience. If we've no previous information of the experience, we need to adapt either our bodies, our minds, or both, so as to gain us through the event... back into our comfort zone once more.

The're several techniques to gain understanding of a given event...
1. We can read about something and gain insight into what happens, and then relate this detailed material to how we 'think' we would cope in that situation. A lot things we read about however, are cleansed of background criteria and do not wholly inform us of the true nature of the picture. Studying is surely not no different as actually experiencing something first hand, however it's certainly superior than having no info whatsoever!

2. We can master about something from a third party who has experienced something, find out about their previous experience, how they coped with things, and then relate this again to how we 'think' we would react in a similar circumstance. It's a helpful technique of learning, however it does not actually relate to how we would manage the situation as the individual(s) we listen to will usually have different life experiences than ourselves and would therefore have reacted to the situation differently. 3. We can essentially experience something first hand, see how it feels for us and then be taught from the experience, whether great or bad. This technique however, is dependent upon how we're able to cope amongst situation, and the way we choose to react to it. A lot individuals can and do, locate this first person experience very stressful depending upon what the experience actually is and also according to their previously learnt coping mechanisms and personalities. Given the option, I am absolutely certain the vast majority of us would avoid a lot of the first peson learning tactics if they involve acute stress and / or pain... i know i'd.


4. We can be instructed by somebody about an experience, informed about what actually happens, at what point something happens, why something happens and what the outcomes of that thing happening actually are. Such learning techniques are perhaps a tiny bit more useful to us as we can better formulate a picture of what shall be happening to us. We also have a lot lot more detailed material about our situation... background info which enable our senses to develop this into a more rounded and constructive picture.

The more relevant info we've, the better prepared we're.


Obstetric Info
Females in labor, their partners and/or significant others must be given sufficient info concerning their specific labor so as to gain the ability to formulate a detailed picture of what will, and can be likely to happen to them throughout their labor.

Such info needs to be relevant to their individual labor and also have enough info to allow them to create 'informed choices' with regards their labor care.

The potential difficulty is how much info ought to they be given and when?

If they're given too little info, they're not wholly informed about their labor. Provide them too much and well, much of it is certain to possibly be forgotten anyway.
What info ought to they be given... only detailed material about their current stage of labor or info regarding the whole of their labor, whether actual or possible?
Traditionally, and from my own experience, the pregnant woman often recieves her info regarding labor in a somewhat haphazard way
When she first registers her pregnancy, she largely recieves basic detailed material about where she will have her baby, together with a number of health promotional info., and perhaps contact details. The baby can be planned for delivery in a local birthing unit or hospital, or it can be planned to happen at the female's own house. Further along the pregnancy continuum, the female can be given extra, updated info as and when this is required. Such a scenario can be when her pregnancy changes from a 'low risk' pregnancy, to that of a 'high risk' pregnancy.

In such instances, she can be strongly advised to have her labor in under the controlled and monitored environment of a clinical setting... plenty of highly trained staff lurking about! This would be particularly so ought to she require a planned cesarian section for a number of reason, or require her labor to be 'began off' (induced).

Females undoubtedly read about labor experiences in books and magazines, and also meet individuals who they can discuss labor with... whether aquaintences or health professionals. These all assist the individual woman to formulate a picture of how her labor will likely be. Thus the female can begin to develop her potential private adaptations for the event.



How much info to provide, and when to provide it

Health professionals all face the dilemma of precisely how much info to provide the individual, and this is the majority of certainly true in under the specialism of obstetrics.

Do the health professionals only provide enough info to the laboring woman and partner to maintain them informed about their current stage of labor, or do they provide them with detailed material on what may happen somewhere down the line?

On the one hand this can be deemed a great thing as giving info which isn't currently applicable can either be lost from the patient's memory (for the reason that it's not important enough at that particular time), or it can cause an individual to begin worrying unnecessarily as they're 'slapped' with info they were wholly unexpecting!

Nor do health professionals particularly desire to scare their customers with info which can be cause for worry when it is not currently relevant... tends to make them fear something is wrong when it is not necessarily!

Very interesting I am absolutely certain... however what is this got to carry out with you?

Well, perhaps one of the main things I learnt during my obstetric practice was the truth that a lot females and their birth partners, truly are not prepared for their labors insofar as what actually happens during their labor, and what may happen throughout the course of their labor!

I've heard obstetric staff only inform the couples the 'routine' stuff... that which is happening 'there and then'. When a potentially problemmatic sign or symptom occurs, they only tell them what has happened, and what they will do about it. Immediately this is I suppose, a perfectly okay solution. After all, staff are informing the individual what is happening and what they plan to carry out about it, however such events are only so reactionary do not you think?

I presume that a large percentage of healthcare professionals do not like 'informing' their customers too much by giving them a large amount of info as they can push them over into 'panick mode'... and I suppose that they would have a point here.

On the one hand, I suppose this could be considered a great thing in a way. 'Great' insofar as the giving of info which isn't currently particularly relevent poses the question as to whether it's actually retained by the individual who can well be currently preoccupied, or alternatively perhaps, a number of health professionals do not particularly desire to scare their customers with info which can be a cause of worry for them.

Individuals contend with detailed material on diverse ways... a number of 'freak out' the minute they hear the detailed facts, whilst a number of consider it for a while and then 'freak out!'.

Theres not a thing worse than being very busy with several customers on the go, and then having one of them 'freak out', have faith in me! It changes your priorities, changes your workload and constitutes extra input when you're able to be 'rushed off your feet' in the 1st place.

And also, a lot staff only simply do not have the time to sit down and fully inform, advise and answer all potential questions!! Okay, you can get a number of staff who inform... others who provide great sound advice, and yet others who answer questions, however you do not always get the entire lot in any 'joined-up' fashion!

Due to healthcare pressures and caseloads, you're often coming into get in touch with several health professionals over several hours or days... and they may be often so busy, perhaps not having the time nor the inclination to only sit down with one or 2 individuals and simply 'talk to them'. Info can often be recieved by the customer in a 'piecemeal' fashion... a bit here, a bit there, and oops, a contradiction there!!

A Personal Involvement... Scary Stuff Indeed!


When my wife was in labor, I was in the unit and labor room with her, Immediately I also 'understood' this labor thing (after all, I would done the teaching!!), however I was definitely mortified when the three of us nearly went for a C-section.

This happened three whole times as I recall in under around a 2 hour period... all due to our unborn son's heart rate falling, and staying low for lengthy periods! He had gotten himself seriously distressed on 3 occasions during the course of labor, and seriously stressed everybody out.... the obstetrician, the staff and his father (his mom was only too 'stoned' to notice!!).

Immediately I will strongly say that I saw this coming, as I had been following our labor from a professionals' point of view also including a prospective parents' view. I was aware of preceding events, I witnessed the signs, I saw the falling heart rate when it began to fall... get lower and slower..., and I knew what stage of labor my wife was at... and it could just have been a C-section contingency plan, there was simply no other course of action to take to resolve the 'crisis'... it was a case of 'All Hands On Deck!'

Immediately I mentioned this having happened 3 times... three whole 'rollercoaster' rides... we were going, then we were not! Then we were, then we were not. My Poor Heart!!

Luckily (3 times so!), our Son's heart rate was to at long last come back up to a decent rate, so averting the trip for the emergency C-Section. My wife had been 'prepped' for theater 3 times, the staff had rushed around like possessed beings 3 times, and I had packed and unpacked our bag 3 times!!

My adrenaline had coursed around my body 3 times... I was so mentally exhausted, and it was not even me who was in labor!

Luckily for my wife, she had been given analgesia for her labor, and could not truly care what happened at those points in her labor. However me! I was definitely stressed out... I feared the worst for my wife and my unborn child, it was definitely horrible and I would not wish the feelings on anybody.

Immediately then, can you visualize what it would this scenario would have been like for the Individual who had no true knowledge or experience of what happens when a C-section needs to be performed 'Stat'?

Individuals who've heard of the C-section, however do not truly know what it entails... the absolute urgency of the situation on occasion, the preceeding preparation, the potential for panic, the LARGE stress levels involved for all involved.

Also including C-section, the're other instances in which prompt and successful treatments are required with the aim to extract a compromised fetus during labor, or to treat an exhausted mom-to-be. Other emergency situations which must be dealt with once they occur... a number of quicker than others depending on the circumstances arising.

Immediately, obstetric staff are aware of what is going off during the course of a labor. They know what they can expect once the precursors begin to reveal themselves. They read the vital signs, gaze at the labor as a whole, and they take, read and interpret the technology.

The're occasions when they can look at a particular 'labor' or 'labor pattern', and be fairly sure that that labor will turn out in a given way.

However do they always tell their customers of their concerns as they occur? Do they always provide them enough advance info of events in purchase for the customers to gain the ability to ingest, process and manage the info? Are they giving the detailed facts which matches the likely labor outcome, or are they sometimes only 'springing' it on these individuals as and when their labor turns into a crisis? The customers are often only 'passively' involved in the course of events!

Whilst they were informed of the 'Generalities' of what labor constitutes, and what to expect during their antenatal classes (until they disappeared through lack of staff and cash), they were still remarkably ignorant of much of the minutiae of it all.

A lot females and their partners were not, I believe, as prepared for their labor as they could have been. They simply did not have all the material facts that they could have had prior to the event occurred (not only C-section). Yes, they had had a number of info (to a greater or lesser degree) on what labor was about, and what happened.

A number of individuals had even been told about things that may happen only when they were least expecting them to happen. However a lot were not... they had not been told!!

So, when the proverbial 'hit the fan', as it often can do during a female's labor, they were given a fast explanation of what either had, or was happened, and then given a shorter or longer briefing (depending on the nature of the event) of what was going to be completed to remedy the situation.

Both the female and her partner would then be left to manage this detailed material whilst everybody began rushing around preparing for this particular emergency event. It ought to have been definitely horrendous to have been given this detailed material all of a sudden, then only be left to manage things... often with no a suitable and proper explanation.

Not ideal, however there you go, such is life I suppose.
Is there an improved way perhaps?

What if these individuals were given the detailed facts about their labor prior to they found themselves in labor?

They would then have enough time to digest the detailed facts prior to the event. Not just would they discover regarding the 'normal' labor and childbirth, however they could also discover regarding the 'abnormal' labor and childbirth... when things go awry, and do not rather turn out as had been expected!!

Whilst a number of would most likely say 'Heck, you do not desire to be telling individuals about horrible and abnormal events which they can experience at such a time!', I'd counter the opposite. A proven fact....

Immediately, it is an established, researched fact that individuals who're given great and sufficient detailed material on a particular issue, are more probable to fare well and be better prepared in both meeting and contending with that particular issue.

Taking a medical setting for example, individuals who've been given sufficient and relevant detailed material on their condition, their treatment and their likely outcomes have been shown to respond better to their treatments and recovery.

I presume this is due to the truth that the individual has all the pertinent detailed material on their given scenario and therefore are in an improved position to comprehend that which is relevant to them at that time.

He or she is then able to mentally prepare for future or current events. It's much superior than finding themselves right bang in the middle of an event and having to manage it there and then! This often creates acute stress, can get the individual panicking, and isn't a great response at all... firstly for the patient, secondly for their significant others, and also for the staff looking after them!

It can be having enough info for a holiday, for a mint condition auto, a certain 'widget', or the care you're recieving when you're in hospital or a situation with which you're unfamiliar!

It helps to know as much as likely about something prior to you either a) Purchase it, or b) Locate yourself in the middle of it!

'Normal' Labors just occur Retrospectively

It's often said that 'normal' labors... that's, those in which a female has labored prior to having had a vaginal delivery, can just be classed as 'normal' after the event.

Every Labor is dissimilar...
A number of females can begin to have contractions first, others can have their 'waters' break first. Females do not realise that their 'waters' have broken, they do not realise they're having contractions and are in an advanced stage of labor... Still other females can not begin to labor at all until a number of nice Obstetrician sees them and starts it off for them! Seriously, you would not believe the variety of diverse things which do and do not happen with girls and their labor!

Difficult for guys too... Menfolk do not always have an simple time of labor either... contrary to popular belief!!
Immediately a number of guys are present during their partner's labor in mind... however not in spirit! They're there out of an expectation rather than a wish to be there, Actually, a number of them could not provide a monkey's toenail about labor or what their partner is going through. They're totally indifferent to it all, and would most likely have been better off staying away and letting somebody who was bothered attending... There, I have said it!
Others are there as an extension of their partner. They desire ot know only about everything thet is happening... the technlogy being used, the stage of labor, the contingency plans... everything.
I've observed men during labor who've been definitely bereft of any self-control... they have simply been unable to comprehend the gravity of the situation. They lock themselves down to a chair in the corner of the birthing room, and feign a disappearance! Anything to steer clear of being spoken to or having to do whatever thing... "Hello, is there anybody hiding behind that newspaper?"

Other guys become upset when they see their beloved apparently suffering the pains of labor. Unfortunately, not all labor pains are always alleviated.

Occasionally, the analgesia won't do the trick, and the labor pains continue unabated. Immediately this is very distressing, not only for the female experiencing them, however for her partner who is witness to their close family and friends' continued distress and obvious discomfort.

This is such an agonising sight to witness as both parties are no longer in charge of events. Whilst the female has other things to contend with, her partner is usually 'beside themself' with grief as they watch their close family and friends' contorted facial expression, and her screams of agony... and they feel definitely powerless to assist!

Immediately, what if individuals were informed well in advance of labor only what happens during the event itself?

Do you believe they would have an improved understanding of what happens during their labor, and indeed, how come it is happening?
